• Breed Survey Form (PDF) to enter a Breed Survey please complete and return to the Club 7 days before the survey with documentation as outlined on the form.

• Hip Dysplasia and Elbow Dysplasia Form  (PDF) send completed form, payment and SAE for return of form to the State HD/ED Registrar at least 14 days prior to the date of the planned X-Ray
